Non-surgical Treatment

In the early stages of PTTD, nonsurgical treatment is preferred. Rest and immobility, physical therapy including calf muscle stretches, single-limb heel rise, and Achilles tendon lengthening stretches, pain relievers in the form of nonsteroidal anti-inflammatory medications (NSAIDs), shoe changes with more arch support, walking boot, and an ankle brace are all options. These procedures try to relieve ankle pain, reduce inflammation, and prevent illness from worsening.

Surgical Treatment

If non-surgical treatment does not improve symptoms, or if the illness is advanced and shows degenerative changes to the medial malleolus or severe deformity, a posterior tibialis tendon surgery may be required. The specific method will be determined by the stage of PTTD and the patient’s anatomy. Tendon repairs or transfers, osteotomies (cutting and relocating bones), joint fusions, and implants to support the arch are all possibilities.
